Online eTravel System

The first time you use the system, you will be asked to complete a form with your team information.  Once this is done, you will be given a password to use in the future.  You will not need to provide the information for your team again as long as you remember your password.  Please be sure to make a record when you first sign up.

Online Travel Notification:  used anytime your team is traveling outside the state of Kentucky or playing in any match that the opposing team is not a member of US Youth Soccer.

Online Guest Player Notification:  Used anytime your team is traveling outside the state of Kentucky or playing in any match that the opposing team is not a member of US Youth Soccer and players are needed for the match that are not listed on the team Roster.

Online Host Notification:  used anytime your teams are hosting games for teams that are not members of Kentucky Youth Soccer but are members of any US Soccer Federation member.

Online Non-Member Play Notification:  used anytime your teams are playing games against teams that are not members of Kentucky Youth Soccer or any US Soccer Federation Member.

The forms must be completed in their entirety.  Upon completion of the forms you will be given a confirmation number and a copy of the completed form will be displayed on your screen.  You can go back at anytime and display or print these forms.  No one can make changes to these forms.  Also at completion of the form, you and your club will be emailed a copy of the confirmation for record keeping purposes.

Your club should keep a copy of these forms, rosters and guest player forms on file for at least 5 years.
